Overview
To provide skilled nursing interventions in the treatment of the patient illness, rehabilitative needs and preventative care. Utilizes the holistic approach in the provision of problem specific therapeutic interventions, teaching and training activities in accordance with the plan of care. Problem specific interventions oriented to achieving outcomes are reflected in his/her ongoing documentation. Responsible to apply professional knowledge and skills in accordance with accepted standards of clinical practice, to facilitate problem resolutions and the achievement of individualized patient goals and outcomes. Practices under the direction and supervision of the registered nurse to proactively meet the needs of and assist the patient in the achievement of individualized, defined and measurable outcomes.
Qualifications
Minimum Education and Experience Requirements:
Licensed Practical Nurse (LPN). One year of clinical based experience, with specialty of home health preferred. Successfully completed 30 hours IV Therapy Course. Demonstrates willingness and ability to maintain clinical competency as required by assigned unit.
Motor Vehicle Operator Designation:
Employees in this position will operate vehicles for an assigned business purpose as a 'frequent driver'. A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job.
